whenever I click on any request to fill up, it leads me to a 404 error page. why is this happening???confused.gif

u have to add this http://prntscr.com/q1jx69 before submit link then add actual link like this http://prntscr.com/q1jxbt. u must have to add http://prntscr.com/q1jx69 this before actual link.
